What is a “Request for euthanasia”?

Most clinics have a document called “Anhållan om avlivning” (Request for euthanasia) or similar. The owner signs it before the euthanasia (often already at the clinic’s reception).

It is a legal document to ensure that it is the owner, and no one else, who is having the animal euthanised.

The Swedish Board of Agriculture requires transport documents where disease control checks are needed.
For Guldveterinären, as a travelling home vet, the document also serves as consent for me to carry out the subsequent transport to the mortuary.

At the same time, the document confirms in writing which type of cremation is wanted.

Guldveterinären always brings the “Request for euthanasia” document to visits, but some owners like to prepare by filling it in before the visit.

If a “Power of attorney and request for euthanasia” is needed, I can print it and bring it if I am told in advance.
