Life certificate

Here you can fill in and print the life certificates of the most common insurance companies.

Please note that most euthanasias do not require a life certificate. For ordinary reimbursement of veterinary care for illness or accident, and when cancelling the insurance, the invoice/receipt is normally sent to the insurance company. Always check with your insurance company what applies in your animal’s case.

If you have taken out life insurance in addition to ordinary veterinary care insurance, this will be stated in your latest insurance documents. Log in to your insurer’s “My pages” to check your animal’s current details.

Some insurers have certificates that must be ordered via their customer service. Others have files to download or fill in on their website. However, they often accept Agria’s general life certificate (see below).

Sometimes the insurer’s approval is needed in advance for the euthanasia to be reimbursed. Note the name of the insurance employee who gave the approval.

The life certificate has two parts

One part is filled in by the owner before the visit. The other part is filled in by the vet at the visit, while the animal is still alive.

Try to keep it brief. Have copies of medical records from previous vets available. Dates and names of the treating vets are often needed. You must be able to state when the illness or injury occurred.

For some diagnoses and circumstances, insurers require a post-mortem of the animal by a pathologist. In that case, the owner must transport the body to a larger animal hospital, where the aftercare and type of cremation are also chosen.

Don’t hesitate to call your insurance company well before the booked visit if you have any questions!
